  • TRT/ ERT protects against brain aneurysms - a team from Rush University Medical Center found that HRT and oral contraceptives reduce the risk of the formation and rupture of brain aneurysms in women.

Diabetes and Testosterone Replacement Therapy

Testosterone Replacement Therapy for Diabetes:

After World War II, testosterone therapy was used to help the holocaust survivors stay alive. It helped them recover from the extreme wasting of the Holocaust.

A general practitioner in Denmark,  Jens Moeller, M.D., made the discovery that testosterone injections were able to reverse many of the signs and symptoms of diabetes for both men and women. We have reproduced some of his work.

 

Why?  The body runs on energy.  it is testosterone that fuels the mitochondria in every cell in the body.

 

Testosterone and Insulin Resistance:

The term 'insulin resistance' to describe the underlying pathology of diabetes is wrong. The role of insulin is to help move glucose (sugar) from the blood stream into storage in the cell (glycogen). Insulin assists the energy pump (facilitated transport) which does 90% of the work.  For example, an adult borderline diabetic male will have a fasting insulin of greater than 10miu/ml. This is how much  insulin is needed at rest. Adding back testosterone (injection/ pellet) reduces the fasting insulin to less than 3miu/ml!  Usable or bio-available testosterone is key to glucose control in all.  

When doctors add back testosterone replacement therapy (TRT) to diabetic men good things occur:

Diabetes: Insulin and glucose are very caustic/ corrosive to the blood vessels and cells. Too much of either deposits the glucose in the cell wall as carmelized 'goop' that jams up the cell. The damaged cells do not function properly. When this is in the eye, it causes cataracts and blindness.  When this is in the kidney, it destroys the filtration mechanisms leading to dialysis, when it is in the heart's coronary arteries, it leads to heart attacks.

  • It is thought that using TRT, at least in part, reduces and can possibly clear some of the Accumulated Glycogen End-products  (carmelization). Doctors find that testosterone replacement can reduce both blood glucose in the blood (glucose level) and in the red blood cell (hemoglobin A1c). A drop of 2% can be seen in moderate and  severe diabetic men; a 2% drop in HgbA1c equates to a  70% drop in complications from diabetes!

  • Testosterone reduces the risk of low blood sugar by improving movement of glycogen from in the cell back into the blood circulation. We are studying how to make life both better and safe for our diabetic patients.

  • TRT reduces heart failure and heart attack risk - males with the greatest level of bio-available testosterone show a dramatically lower risk of heart attack and heart failure, a team of Danish researchers reported in the British Medical Journal.  [1]   

  • Testosterone dilates the coronary arteries as well as increasing blood flow to the penis with sex.

  • One form of our TRT mixture when added to metformin may help protect against blood clots and deep vein thrombosis. This has been known for 50 years. 

There are so many benefits to raising your natural hormone levels to that of a young adult. We may not be able to make ourselves 25 years old again, but, having the hormone levels of a 25-year old maay well be the first step in that correct direction.

Diabetic medications are still used!

  • Metformin: reduces glucose absorption in GI tract

  • Insulin: human formulation is cheaper and just as effective as man-made insulin. Small amounts are used to improve glycemic (sugar) control in the more severe forms of diabetes.

What to Expect:

in 1999, Lichten and Sowers showed a 50% reduction in insulin requirements in the most difficult to control brittle diabetic men using more than 100 units of insulin daily.[5]

Most men on oral agents were able to discontinue all, but metformin at a $300/ month cost saving and lose weight.

Harvard's EL Ding[2] 8 years later wrote " all diabetics are testosterone deficient", England's D. Kapoor [3] at the Sheffield Andrology Center in 2007 showed that an injection of testosterone every other week dropped men's HgbA1c (glycogenate hemoglobin)  by 2% and Bayer Pharma in 2007 published a 52-week study [4] using their every 3-week Nebido injection with similar results.

Introduction: Biography of Edward M. Lichten, M.D., F.A.C.S., F.A.C.O.G.

 

Edward Lichten is a 70 year old board certified physician who was mentored by the legendary gynecologist Frederick P. Zuspan,  M.D. Professor Emeritus at Ohio State University.  Trained in the most advanced surgery for cancer, Dr. Lichten chose to pursue a search for the medical imbalances that contribute to and cause disease.

 

Background Information: "Anti-aging" and Disease Research: 
Since the 1960's, gynecologists treated menopause with the addition of estrogen and testosterone to revive 'life'; and 'energy' and sex drive. Until just 20 years ago, no one connected imbalances of 'hormones' with non-gynecologic disease states in women such as PMS has  menstrual migraine, nor symptoms in men such as low testosterone (men'pause), diabetes and inflammatory diseases. 

 

Lichten's Research since the 1980's:

 

Dr. Lichten discovered in the early 1980's that testosterone replacement could do wonders for diabetic men.

Testosterone Reverses Need for Diabetic Medication: As his women patients saw the difference testosterone made for Dr. Lichten, they demanded their husbands be seen for the miraculous 'fountain of youth' hormone treatments. The third man to be seen, Joe. N. was a 295 pound, 5-foot 9-inch male with diabetes. On testosterone, he lost 85 pounds in the first year and discontinues all his diabetic medications in the first month. Now, 24 years later, Joe. N. is still not clinically diabetic.

Research: Since the 1950's, Jens Moeller, M.D. in Denmark treated both men and women very successfully with testosterone injections. This treatment was downplayed and ignored by those uneducated in the use of testosterone (especially in women- note as a gynecologist I have used testosterone for women for 45-years).  Then EL Ding at Harvard[1] showed that all diabetic men were low in bio-available testosterone.  However, major andrology centers in England  [2,3] and Switzerland [4] have documented that testosterone reduces glycogenated hemoglobin(Hemoglobin A1c) by 2 %!  Diabetic medications are the most expensive by total cost of all pharmaceutical medications making the financial aspects of using less than five dollars ($5) of testosterone per week for men and less than one dollar ($1) for women a 'bad financial' practice for pharmaceutical and insurance companies. 

 

When looking for a cause of disease, what has been overlooked are lack of bio-available Hormones! Main stream medical research had failed to consider that many chronic inflammatory diseases are mediated through excesses of estrogen and low levels of testosterone. Starts with diabetes. Diabetes and obesity are linked to hormones. Just treating the low testosterone levels will improve diabetic control, help with weight loss, improve muscle and sexual performance. Men on testosterone are happier and have improved mental clarity. Recent studies show that men on testosterone injections (not topical because they convert to estrogen) have less heart disease, less hospitalizations and less Alzheimer's.

Youthful Hormones!  That is why a 50-year old's broken bones take longer to heal than a 15-year old's. When we add back the natural bio-identical testosterone and nandrolone, he/she can stay more alert, involved, active and recover faster--LIVING BETTER and LONGER!

 

The Most Radical Breakthrough is using Testosterone more freely to Treat Men and now Women for Disease!
In the early 1980's, Dr. Lichten experienced what is now called andropause; 'men'pause for men. Night sweats, fatigue, erectile dysfunction, weight gain and depression became his way of life.  When searching for a hormonal cause, Dr. Lichten discovered that the laboratory blood tests explained the disorder: men were experiencing a men' pause (equivalent to a woman's menopause).  All of his symptoms disappeared and he was able to build muscle tone, mental acuity, and endurance not realized since his 30's with small doses of natural, bio-identical injections of testosterone and F.D.A. approved anabolic steroids!

 

The Clinical Research:
Buoyed by the anti-diabetic effects of testosterone is a multitude of men, Dr. Lichten enlisted James Sowers, M.D., Professor and Chairman of Endocrinology at Wayne State to co-sponsor a university approved clinical study. [5] The results proved that:

1) all diabetic men are low in testosterone,

2) adding back testosterone would reduce the need for insulin by 50% and oral agents nearly completely, and 3) the men on testosterone did not report any low blood sugar complications. A large clinical study using more potent testosterones is being planned as clinical research from Harvard, England and Switzerland support exactly what Dr. Lichten published to the Internet 18 years ago.

 

Putting It All Together:
As the author of the Textbook of Bio-identical Hormones, Dr. Lichten has put together some of the missing pieces that  underlie most chronic disease-- hormone imbalances. These hormones can be measured in common laboratory blood tests (See Booking an Appointment). Vitamin D3 deficiency is links to insomnia, low growth hormone to fibromyalgia, thyroid to hair loss, fatigue and weight gain, adrenals to afternoon fatigue, pancreas to diabetes and digestion. My worst case of insulin requiring diabetes was a 25 year old African-American, homeless, drug addicted male whose sister held an important position with the Detroit major's office.  He had a random glucose of over 400mg /ml, Hgb of greater than 18,  SHBG over 70 nmol/l. I started him on 200mg of testosterone cypionate intramuscular weekly and then add 60mg of nandrolone and then 25mg of stanozolol to his weekly injection. Within 8 weeks, his random glucose was under 200mg/ml and his HbgA1c had dropped to 12.3.  At 9 months he reached a HgbA1c of 17.9 and was working at a Anthony his addictive drugs and was lost to follow up and died two year later."  Edward Lichten, M.D.

Jon P. came to me as an obese 40ish white male weighting over 330 pounds with Adult Onset Diabetes; HgbA1c 8.8. His other Metabolic Syndrome findings included hypertension, and high cholesterol.  His initial laboratory testing showed a testosterone total 190ng/dl;SHBG of 70nmol/l.  His energy level improved on  200mg testosterone cypionate, 60mg of nandrolone and 25mg of stanozolol as weekly injections. He lost 70 pounds, and although a big man,  his Hgb 1c is well within normal range, his total cholesterol dropped and he is on just metformin and low dose lisinopril.

Similar case reports are found in the long-term management of adult women with Type II/ Adult onset Diabetes.

The management of diabetes in menopausal women follows the protocols used in men but omitting testosterone and lowering the dose of nandrolone and stanozlol in half. 

 

 However, long-term therapy finds that women who have been diabetic for years and those not menopausal do have more risk of progression of cardiovascular and coronary artery disease. Replacement of DHEA is a very important consideration along with metformin.
